Service Information:
Somerset Supporters provide help and support to children and young people up to 18 with severe learning and physical difficulties.
They work with an individual in a group, in the community or at one-to-one meetings.
Support can take place in the family home or through meaningful activities in the community.
You will have an assessment before a decision is made about whether you can get access to the service.
When the worker refers you into the service, they find a suitable supporter to be matched with you. Then they draw up a contract to confirm the hours and planned activities.
